Bangkok (3 days)
Chiang Mai and a 2-day 1-night trek that includes rafting (1+2 days)
Chiang Mai to Yogyakarta (multiple flights, 1 day total travel time)
Yogyakarta as a base, with day trips to Borobodur and Prambanan, and a
volcano climb (3-4 days)
Bali (3-4 days)

Rushed, but doable.
