Oops, I just read the OP again and it seems like there is no onward
travel to the US. It appears that the US citizen is currently living in
Canada, and wants to go to the DR and return to Canada.

In any case, a US *passport* is required. Make a trip to the closest US
consulate in Canada to apply for one. Of course, there's a chance that
the Canadian immigration official will just wave him through with a birth
certificate and photo ID, but why take a chance? The OP could face, at
worst, deportation. Get the passport.
