And to go further into it, the metroliner was told to go into
position and hold at an intersection. the USA 737 was told to land, and
didn't controller didn't realise he still had the departure in
position. Result of that was an order by the FAA to no longer have
departures in position and holding on the runway at night.

none of these are *runway* incursions. LAX has had a few of
them including the semi-famous one where a Brasilia was told
to hold short and instead went into position and held. A
southwest (?) jet landed on top of the twin turboprop. These
are 'mere' taxi incidents.
