I heard a report that they suspect the engineer of the commuter train might have been text messaging and not paying attention to doing his job. It sort of argues that engineers should only be backups for remote controls. England's Great Western Railway was doing more or less that in 1906. (The system only sounded a warning and applied the brakes; it didn't shut off steam.)
