Using U.S. dollars (cash), I have been able to exchange directly with
the merchant at times and most of the time with my credit card.

I use banks in europe only to exchange back excess currency back to
Dollars or Euros.

Forex opened an office here a couple of years back. It was amusing to
read their PR material. They claimed to be able to offer cheap cash,
because they bought it in large quantities at low prices, and they were
recommending travellers checks for travel in Western Europe.

That is the most expensive way possible.

That was not part of the criteria, but certainly is a way to exchange
money... as is credit cards.
