Anyone point me to a list of the questions asked on this form, or a jpeg/gif
closeup? I tried Googling for an image, but the only ones I found were in
Japanese or German

Whats more interesting is the idea that someone expects an
individual might answer yes to those questions.

Aw, think about it. That's not what they expect; what they expect is
that the individual will falsely answer no, and can then be deported
or prosecuted for lying on the form.
